Filling in forms and assessments can seem daunting and not worth the effort but with my help, we can make the process easier and more effective.
I can help you fill in forms, whether these are in paper form or online making sure we get all the relevant information recorded. I can also help you prepare for any assessments as well as attend them with you (depending on the location) if required as an additional pair of ears, to ensure you understand everything that’s going on. It is sometimes difficult to take everything in that’s being said to you when you’re also trying to answer questions.
As well as considering social care options, you may find that you are entitled to health related benefits, depending on your situation. If you need help looking at what benefits maybe available to you I can help. I can help you complete the relevant forms, either in paper form or online. If assessments are required, I can help you to prepare for the assessment and attend with you (depending on location) if required. If I am not able to help you I will put you in touch with the right people who can provide the support you need.
